If you can rotate the clevis end by hand and it rotates freely and the arm retract or expend, then the actuator has not seized.
Check the positioner is outputting 30 to 36 Volt when activated.. if no output the positioner is faulty.
Could be a faulty reed switch at the motor. Very common. if you connected sensor cable to the DC output the reed switch will be fried.
Could be faulty cabling. no feedback from the reed sensor.
