Survey is insufficiently anonymized ~ I right-click on link, select 'open in a private window' --> prompts me for my hotmail/office login credentials.

Rationale: others have posted survey links such as this, which can be completed in a private browser window, not requiring any online account login --- this allows participants to be anonymous.

conclusion: if this is not some sort of scam, OP need revise their IT skills (no offense intended =)

The first thing the link requires you to provide is a microsoft log-in.

If this is genuine, it's been very poorly done.

Sounds more like a marketing scam though.
