I am guessing someone didn't do their job right...
I guess not, or all that money spent on engine mods but none on the rest of the car. I see so many guys do all the engine work e.g. my mates HSV GTO, but not the axels, clutches and diffs etc. Then they wonder why is my car snapping axles or why is my diff blown??
no one could be that dumb surely.

It looks like the wheel nuts were loose but most likely something else.
When prepareing a car for motorsport you dont use any old wheels and nuts.
Wheel nuts must have a large flange to prevent pulling through the rims.
Wheel nuts should also be replaced regularly in heavy service. Have you ever looked inside one at the condition of the threads?
Wheel studs should be long enough to alow for the extra thickness of some alloy rims, many stupid people think 5 or 6 turns is enough.
Rims should not be supported by the wheel nuts in performance situations.
They should sit tight on the axle flanges, this will allow all shock load to aviod the nuts and studs and go direct to the flange. The nuts just clamp.
Regulations in any motokahna or track days the wheel nuts must be exposed (no caps covering nuts) so the scruterniers can check that your wheel nuts are on before you go out.
Sounds like someone with no idea decieded to go out to a track day......
